Medbiz obtains its products from numerous vendors and distributors.
As a result, the Terms of Sale and Return Policies depend on where an item originates. Refer to your invoice to obtain the vendor
or distributor information. Here are the Terms of Sale and Return Policies for our vendors and distributors:
Curascript
RETURN POLICY
Curascript will accept returns and provide 100% credit to the customer when returns are initiated for the following reasons:
- Wrong product or quantity shipped.
- Product damaged upon receipt.
- Product dated at 6 months or less, unless customer was notified of dating prior to ordering.
- Manufacturer recalls.
To insure 100% credit, please call Medbiz's customer service department at 888-827-5477, 8 a.m. to 5 p.m. (PST) within 48 hours to authorize the return and arrange a pick-up for the returned good. ALL RETURNS MUST BE PRE-AUTHORIZED BY CURASCRIPT AND MEDBIZ LLC.
Products must meet all requirements as established in the Prescription Drug Marketing Act.
If a customer initiates a return for any reason where Priority Healthcare is not at fault, the following conditions apply:
- Product must be sellable (defined as clean, unmarked, and in original packaging).
- Medbiz must be notified and complete pre-authorization of the return within 30 days from the date of invoice.
- Product must have at least 6 months dating.
- The product may NOT be a refrigerated item.
- The return will be subject to final inspection and approval by Curascript management.
- If credit is granted, a re-stocking fee and shipping charges will be assessed. The re-stocking fee will be 10% of invoice amount. Shipping charges will be dependent on geography and shipping method.

RETURN POLICY
GUARANTEED SATISFACTION - If you have tried a product and it is defective or does not perform satisfactorily, we will provide a credit,
exchange or refund…it's your choice. Simply call Medbiz's customer service department at 888-827-5477, 8 am to 5 pm (PST), within
30 days of invoice date to receive a Return Authorization Number. The merchandise must be returned in its original container, unmarked and
properly packaged, along with a copy of your invoice and a description of the problem.
HENRY SCHEIN WILL NOT ACCEPT ANY RETURNS WITHOUT PRIOR AUTHORIZATION.
Please note these exceptions:
- Equipment: Opened and used equipment may not be returned for credit. Before opening equipment, we suggest that you check the
shipping container and packing list to ensure that you are getting exactly what you ordered. Equipment is backed by the manufacturer's
repair or replacement warranty. Please read and return all warranty information required immediately upon taking delivery of your new
equipment.
- Opened Computer Hardware and Software: Not returnable.
- Special Order Items and Controlled Drugs: Not returnable.
- Rx Products and Controlled Substances: Regulations require us to limit the sale of Rx and controlled substances only to registered,
licensed healthcare professionals. If you are a new customer, please furnish us with your DEA number and expiration date during your
registration.
- Items that cannot be returned to manufacturer for credit are not returnable.
- You will receive 100% credit for all products returned within 30 days from invoice date (except on exceptions noted above).
For products returned after 30 days from invoice date, a 15% handling charge will apply.

Officemax
RETURN POLICY
If you are not satisfied with your purchase, simply return it to Officemax in the complete and original packaging (together with all
accessories and manuals) with your original sales receipt within 30 days of the date of purchase. We will gladly replace or repair the
item, or return your money. Electronics items, digital cameras, computers, PDA’s and business machines must be returned within 14 days
of purchase. OfficeMax.com will not accept returns of opened computers, PDA’s, digital cameras or software packages unless defective.
Software and digital cameras that are defective may be exchanged for identical items only.
You must contact Medbiz's customer service department at 888-827-5477 to obtain an Authorization to Return (ATR) and to request a
pickup of the item.
